Imipramine Hydrochloride (TOFRANIL) is primarily used for the treatment of major depressive disorder. It may also be used for enuresis (bedwetting) in children and off-label for anxiety disorders and chronic pain management.

Dosage Information
Adult Dose
The typical starting dose for depression is 75 mg per day, which may be increased gradually to 150-200 mg per day. The dose can be administered in divided doses or as a single dose at bedtime. The maximum dose should not exceed 300 mg per day.
Pediatric Dose
For enuresis in children aged 6 years and older, the starting dose is 25 mg daily, usually taken one hour before bedtime. The dose may be increased to 50 mg for children aged 6-12 years and up to 75 mg for those over 12 years, if necessary.
Side Effects
Common side effects include dry mouth, constipation, urinary retention, blurred vision, dizziness, drowsiness, weight gain, and increased heart rate. Some patients may experience orthostatic hypotension and sweating.
Contraindications & Precautions
Contraindicated in patients with hypersensitivity to imipramine or other tricyclic antidepressants, recent myocardial infarction, and concurrent use with monoamine oxidase inhibitors (MAOIs). Caution is advised in patients with cardiovascular disorders, glaucoma, and a history of seizures.
Important Warnings
Imipramine may increase the risk of suicidal thoughts and behaviors in children, adolescents, and young adults. It should be used with caution in patients with a history of bipolar disorder due to the risk of inducing mania. Monitor for signs of serotonin syndrome, especially when used with other serotonergic drugs. Avoid abrupt discontinuation to prevent withdrawal symptoms.
