Uses & Indications

Ciclosporin (GENGRAF) is used for the prophylaxis of organ rejection in kidney, liver, and heart allogeneic transplants. It is also used in the treatment of chronic rejection in patients previously treated with other immunosuppressive agents. Additionally, it is indicated for the treatment of severe, active rheumatoid arthritis and severe psoriasis.

Storage Requirements

Store at 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F). Protect from light and moisture.

Dosage Information

Adult Dose

For organ transplantation, the initial oral dose is typically 15 mg/kg/day, administered 4 to 12 hours prior to transplantation and continued post-operatively. For rheumatoid arthritis, the recommended dose is 2.5 mg/kg/day, divided into two doses. For psoriasis, the starting dose is 2.5 mg/kg/day, divided into two doses.

Pediatric Dose

For pediatric organ transplantation, dosing is individualized based on trough levels and clinical response, often starting at 10-15 mg/kg/day.

Side Effects

Common side effects include hypertension, renal dysfunction, tremor, hirsutism, gingival hyperplasia, and increased risk of infection.

Contraindications & Precautions

Contraindicated in patients with hypersensitivity to ciclosporin or any component of the formulation. It should not be used in patients with uncontrolled hypertension or malignancies.

Important Warnings

Ciclosporin can cause nephrotoxicity and hepatotoxicity, and requires regular monitoring of renal and liver function. It may increase the risk of infections and malignancies due to immunosuppression. Blood pressure should be monitored regularly due to the risk of hypertension. Avoid grapefruit juice as it can increase ciclosporin levels.
