Dosage Information
Adult Dose
The initial dose is typically 100 mg administered by slow intravenous injection over 1-2 minutes. This may be repeated every 5 minutes until the arrhythmia is controlled, up to a maximum total dose of 1 gram. Alternatively, a continuous intravenous infusion may be used, starting at 1-4 mg/min.
Pediatric Dose
For children, the dose is generally 3-6 mg/kg administered intravenously over 5 minutes, with a maximum single dose of 100 mg. Maintenance infusion rates are typically 20-80 mcg/kg/min.

Common side effects include hypotension, bradycardia, gastrointestinal disturbances (nausea, vomiting), dizziness, and a lupus-like syndrome with long-term use.
Contraindications & Precautions
Contraindicated in patients with complete heart block, second-degree AV block (without a pacemaker), systemic lupus erythematosus, and known hypersensitivity to procainamide or any component of the formulation.
Important Warnings
Procainamide can cause potentially fatal blood dyscrasias, including agranulocytosis, bone marrow depression, and neutropenia. Continuous monitoring of ECG and blood pressure is recommended during administration. Caution is advised in patients with renal or hepatic impairment, and dose adjustments may be necessary.
