Dosage Information

Adult Dose

Initial: 5,000 IU IV bolus, followed by continuous IV infusion of 1,000-2,000 IU/hour; adjust based on aPTT or anti-Xa levels

Pediatric Dose

Initial: 75-100 IU/kg IV bolus, followed by continuous infusion of 20-30 IU/kg/hour; adjust based on aPTT or anti-Xa levels

Side Effects

Contraindications & Precautions

Active bleeding, severe thrombocytopenia, hypersensitivity to heparin, history of heparin-induced thrombocytopenia (HIT), and conditions where bleeding risk is high
