Dosage Information

Adult Dose

For DVT prophylaxis: 5000 IU subcutaneously every 8 to 12 hours. For treatment of DVT/PE: Initial bolus of 80 IU/kg IV followed by continuous infusion of 18 IU/kg/hour, adjusted based on activated partial thromboplastin time (aPTT).

Pediatric Dose
