Uses & Indications

Treatment of complicated skin and skin structure infections caused by susceptible Gram-positive bacteria, including MRSA; treatment of bacteremia and endocarditis caused by Staphylococcus aureus; treatment of osteomyelitis.

Storage Requirements

Store at 20-25°C (68-77°F); protect from light; reconstituted solution should be used immediately or stored at 2-8°C (36-46°F) for up to 24 hours.

Dosage Information

Adult Dose

4 mg/kg IV once daily for skin infections; 6 mg/kg IV once daily for bacteremia and endocarditis; duration varies based on infection type (usually 7-14 days for skin infections, longer for bacteremia/endocarditis).

Pediatric Dose

6 mg/kg IV once daily for children ≥1 year; safety and efficacy not established in children <1 year.

Side Effects

Nausea, diarrhea, headache, insomnia, rash, injection site reactions, elevated creatine phosphokinase (CPK) levels, myopathy.

Contraindications & Precautions

Hypersensitivity to daptomycin or any component of the formulation; caution in patients with renal impairment; not recommended for use in pneumonia due to inactivation by pulmonary surfactant.

Important Warnings

Risk of myopathy and rhabdomyolysis; monitor CPK levels, especially in patients on statins; may cause eosinophilic pneumonia; not for use in patients with a history of hypersensitivity reactions to daptomycin.
