Rivaroxaban is used for the prevention of stroke and systemic embolism in patients with nonvalvular atrial fibrillation, treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE), and for the reduction of risk of recurrence of DVT and PE. It is also used for the prevention of DVT which may lead to PE in patients undergoing knee or hip replacement surgery, and in combination with aspirin to reduce the risk of major cardiovascular events in patients with chronic coronary artery disease (CAD) or peripheral artery disease (PAD).
Storage Requirements

Dosage Information
Adult Dose
For the prevention of stroke and systemic embolism in nonvalvular atrial fibrillation: 20 mg once daily with the evening meal. For DVT/PE treatment: 15 mg twice daily with food for the first 21 days, followed by 20 mg once daily with food. For reduction in the risk of recurrence of DVT/PE: 10 mg once daily after at least 6 months of standard anticoagulant treatment. For CAD/PAD: 2.5 mg twice daily in combination with aspirin (75-100 mg) once daily.
Pediatric Dose
Not typically used in pediatric patients. Dosing in children is not established and should be determined by a healthcare provider based on specific clinical circumstances.
Side Effects
Common side effects include bleeding complications, anemia, nausea, and dizziness. Less common side effects may include allergic reactions, liver enzyme changes, and thrombocytopenia.
Contraindications & Precautions
Contraindicated in patients with active pathological bleeding, severe hypersensitivity to rivaroxaban or any component of the formulation, and in patients with severe renal impairment (CrCl < 15 mL/min) or hepatic disease associated with coagulopathy and clinically relevant bleeding risk.
Important Warnings
Rivaroxaban increases the risk of bleeding and can cause serious or fatal bleeding. Use with caution in patients with increased risk of bleeding, such as those with recent surgery, trauma, or concomitant use of other medications that affect hemostasis. Discontinuation of rivaroxaban in patients without adequate alternative anticoagulation increases the risk of thrombotic events. Spinal/epidural hematoma risk is increased in patients undergoing neuraxial anesthesia or spinal puncture. Regular monitoring of renal function is advised, especially in patients with renal impairment.
