Adult dose: | For AMI: 15 mg IV bolus, followed by 0.75 mg/kg (maximum 50 mg) IV infusion over 30 minutes, then 0.5 mg/kg (maximum 35 mg) IV infusion over 60 minutes; for ischemic stroke: 0.9 mg/kg (maximum 90 mg) IV, with 10% given as a bolus over 1 minute, followed by the remainder over 60 minutes; for PE: 100 mg IV infusion over 2 hours
